Completing an Electronic Claim Form

The form has Tabs at the top of each screen. Click the Tabs to move between screens and then complete the required details on each screen. The screens where you are required to key in information on the Product Stewardship for Oil Program are the Details, Benefit Period, Claims and Declaration screens.

Help is available when you are completing each field on these screens. For more information refer to Help when completing the claim form. Once the form is completed you will need to Save and Check the form before it can be sent to the Tax Office.
